Advanced Directive Campaign

In conjunction with the Tennessee Advance Directive Initiative, this campaign is focused on increasing "the number of Tennesseans who have a conversation about Advance Directives, who complete an Advance Directive, and who choose to direct their own end-of-life care."  An Advanced Directive is a living will allowing patient(s) to express their wishes and choice of care at the end-of-life.  Start the conversation now so that your wishes are known. 

Hospice Veterans Program

The Tennessee Hospice/Veterans Partnership (HVP) is a state-wide initiative providing technical assistance and program development recommendations aimed at strengthening the relationships between community hospice agencies and VA facilities to improve veterans’ access to hospice and palliative care.

Pediatric Coalition

The Pediatric Coalition was formed to improve the current delivery of pediatric hospice and palliative care services through collaboration and education; develop state standards relative to the ACA's concurrent pediatric palliative care program to present to the TennCare Bureau and the MCOs for adoption; and assist those hospices with infrequent referrals in caring for pediatric patients with real-time clinical and billing expertise from the Coalition.  


Tennessee Cancer Coalition

The Centers for Disease Control and Prevention (CDC) identifies the National Comprehensive Cancer Control Program as, “developing and providing an integrated and coordinated approach to reduce the incidence, morbidity, and mortality of cancer through prevention, early detection, treatment, rehabilitation, and palliation.”  Special thanks to our Tennessee Cancer Coalition (TC2) partners for the creation of our hospice public service announcement.
